Within several thousands years both instruments and methods of circumcision have changed. The essence of circumcision is confined to excision of a penis' prepuce. It is important to notice that cosmetic result depends too much on a method of circumcision.
The most widely spread methods are as follows:
In this method everything depends on a surgeon, i.e. what, how much and what instruments he uses (for instance scalpel, scissors) as well as how evenly he will suture. This is a "traditional" circumcision in Russia.
At this type of circumcision sutures are not placed. As a rule this method prevails among ritual circumcisions. Very often a rough cicatrix forms after it. This is a cosmetic defect due to wound disruption.
Special devices, such as Gomco Clamp and PlastiBell were elaborated since circumcision started to be performed by surgeons. Plastibell is placed on a balanus. After that prepuce is corded above the Plastibell. And within 4 - 7 days spontaneous prepuce falling-off occurs. This method is frequently used at an early age. But unfortunately sometimes prepuce doesn't fall-off and a child suffers pain for 7 days.
In general several dozens of such devices are known. But the majority of them are more the displays of museums and are not used in medical practice.
I think that any child deserves individual approach. The best from old and new methods is observed in my operation technique. I use a special clamp which provides minimal prepuce trauma and at that there is no risk of balanus injure. Excision of prepuce is carried out by a special electrical appliance. It provides complete hemostasis (arrest of bleeding) and at the same time connects parts of prepuce. Hence the operation is bloodless and good cosmetic effect is achieved without complications.
